The Changing Landscape of GP Appointments in the NHS
Less than half of all GP appointments in England are now conducted by a doctor, as patients are increasingly being seen by other healthcare professionals. Recent NHS data highlights that for the first time, the majority of consultations in general practice were carried out by nurses, pharmacists, and physician associates rather than GPs.
There were a record 329 million GP appointments in England last year where the professional role was recorded. Of these, only 162.5 million (49.39 per cent) were conducted by a GP. This marks a significant shift from a decade ago when GPs and other clinical staff in surgeries were present in equal numbers. Today, GPs are outnumbered by nearly three to one.
The shift in appointment allocation has been driven by a combination of increasing demand for services and a shortage of trained GPs. In response, the NHS has encouraged surgeries to recruit a broader range of clinical staff to help manage patient care. This includes physician associates (PAs), who complete a two-year postgraduate healthcare course after an undergraduate degree.
Despite these changes, patient satisfaction with general practice has declined to its lowest recorded levels. Surveys indicate that many patients face difficulties securing appointments. Exclusive polling conducted in December revealed that only 36 per cent of British adults felt confident they could secure a GP appointment when needed.
Healthcare experts stress that while additional clinical staff play a valuable role in supporting patient care, they should not be seen as direct substitutes for GPs. The Royal College of General Practitioners has emphasised that all healthcare professionals should work within their competencies to ensure patient safety.
The current shortage of GPs, combined with efforts to expand the workforce with alternative healthcare professionals, has led to concerns about continuity of care and potential risks associated with misdiagnoses. Experts highlight the need for a balanced approach that ensures patients receive the right level of expertise while addressing the challenges of recruitment and retention in the healthcare sector.
